What do we mean with ‘Looked
After’ young people?
If a young person is:
- living in a community home and other specialist placement
- in foster care
- waiting to be adopted
- on a
Care Order placed at home with their parents
Young people under other local
authorities’ care receive the same service as those under
Nottinghamshire’s care.
Who will be your teenager’s
Personal Adviser (PA)?
If your son or daughter lives
in:
What service does a Personal
Adviser provide?
Your son or daughter’s PA will see them
at least three times a year. This will usually be in school. If
they’re not attending school or are over 16 the PA could see them
at home, in a Connexions Centre or in a convenient public place.
This could be on a one-to-one basis, a group session or at a Social
Care review.
The PA will complete an assessment form
with your son or daughter. Together they will work out the best
support for your child. We realise that at times your teenager
may need more support than others. We therefore consider them a
high priority and we’ll support them in a sensitive manner. A
record of all contact with your teenager will be kept on the
Connexions database.
Your son or daughter’s Personal Adviser
should be invited to all Social Care and Education reviews. Usually
they will see your teenager before the meeting to find out
what they want to discuss and how the PA can best support them.
What happens when your son or
daughter leaves care?
Your son or daughter will have a PA who
specialises in helping care leavers. This new Leaving Care Personal
Adviser will be able to work with them until they are 21.If
your teenager goes on to higher education the PA will support
them until they’re 24.
