Transition reviews happen every year in
school, from Year 9 onwards, for every student with a statement of
Special Educational Needs and in some schools for students with
‘School Action Plus’.
The review deals with your son or daughter’s
future options and lifestyle. At the review you will meet people
such as a
Connexions Personal Adviser,
head or senior teacher, social worker, educational psychologist,
speech therapist or learning support assistant. Your daughter or
son will also be invited to attend. You’ll be able to give your
opinion on future plans and discuss other issues such as health and
money.
After this meeting a ‘Transition Plan’ will be
drawn up. This can be changed at each annual review, if necessary,
as the young person develops. Some reviews are called ‘Client
Centred Reviews’ where the student is encouraged to help organise
the meeting beforehand, be involved as much as possible in the
discussions and be a central person in helping create an action
plan to help them progress and achieve.
The annual review meeting in Year 11 is very
important. Your daughter or son will be asked to make choices that
will affect their future:
- in education
- in work
- in daytime services.
This can be a difficult and confusing time and
your child’s Personal Adviser will advise you about these
choices.
